Sponsorship and Gift Aid Form

Using our sponsorship form allows eligible donations to be Gift Aided, meaning we can claim an extra 25p for every £1 donated at no extra cost to the donor. Please email a copy of the completed form to BDAA at office@bdaa.org.uk along with the date of the matching donations so that we can claim the gift aid. Alternatively, contact us on office@bdaa.org.uk for our address to post the forms.
